A hospice team that genuinely cares

Who We Are

The delivery of superior care to our clients is our number one priority. We value feedback from our clients regarding the care that they receive while under our service. It is our practice to include our clients in planning their care. We request that if at any time during your hospice service you have concerns or are dissatisfied with any aspect of the care that you are receiving, you notify us immediately. We are nurse owned and operated and accredited by ACHC.

Client confidentiality is always a priority within the agency. The release of client information, when required, will be done in accordance with the procedures for the release of information. In emergency situations, we may have to release your information without your knowledge. Please refer to the Privacy Practice Notices at the end of this packet.

A Peaceful Journey Hospice does not discriminate on the basis of race, color, religion, sex, national origin, age, political affiliation, marital status, physical and/or cognitive mental disabilities, veteran status, or sexual orientation. This practice is also applicable between staff and clients.

Hospice care is focused on each person’s values, beliefs, and wishes, not the disease that has made them a client.

Hospice is:

  • A special way of caring for people who are seriously ill, with the emphasis on comfort (palliative) care and symptom control rather than on curative treatments.
  • A philosophy of care for clients and families who are facing life-limiting illnesses, providing support and care in the last phase of an incurable disease.
  • Hospice neither hastens nor postpones death. Hospice exists in the hope and belief that through appropriate care and the promotion of a caring community sensitive to their needs, clients and their families may be free to attain a degree of mental and spiritual preparation for death that is satisfactory to them.

A Peaceful Journey Hospice is:

  • A proprietary, free-standing hospice agency established in 2021.
  • Medicare/Medicaid certified
  • Licensed by the Colorado Department of Public Health and Environment.
  • Accredited by Accreditation Commission for Healthcare (ACHC)
  • A Hospice agency that serves the counties of Adams, Arapahoe, Boulder, Broomfield, Denver, Douglas, Jefferson, and Weld.
  • A team of caring professionals, focusing on relieving the symptoms of the disease and assisting the family in coping.

Criteria for Admission to Hospice services

  • The client’s attending physician and the hospice Medical Director certify that you have a life expectancy of six months or less.
  • The client/ representative signs an election statement that identifies the services that will be provided by the hospice and acknowledges that the services are palliative and not curative.
  • The client/ representative understands that by electing hospice care, the client is agreeing to give up traditional Medicare benefits to receive the hospice benefit related to the terminal illness.
  • The client has a willing/ available relative or friend that can provide care for them (this may not apply in certain circumstances).
  • Under the Medicare Hospice benefit, the hospice will provide the items, services, and drugs related to the client’s terminal illness and related conditions. The hospice will determine which conditions, items, services, and drugs are related to the terminal illness for hospice.
  • The client/ representative has the right to request a Client Notification of Hospice Non-covered Items, Services, and Drugs Addendum.
  • The purpose of the Addendum is to notify the Medicare beneficiary/ representative in writing, of those conditions, items, services, and drugs that will not be covered because the hospice has determined that they are unrelated to the terminal illness and related conditions.
  • The hospice MUST furnish this addendum within 5 days if requested at the time of Hospice election and within 72 hours if requested during the course of hospice care.
  • As a Medicare Beneficiary, you or your representative have the right to contact the Medicare Beneficiary and Family-Centered Care-Quality Improvement Organization (BFCC-QIO) to request immediate advocacy if you or your representative disagree with the decision of the hospice agency on items the agency has determined to be non-covered and unrelated to the terminal illness and related conditions.

Scheduling Visits

Scheduling of appointments is coordinated between the client and the assigned clinician. If you are unable to be home when a hospice visit is scheduled, please notify the agency as soon as possible.

It is our goal to schedule visits at convenient times for our clients. Changes in scheduled visit time or assigned clinician are sometimes necessary for a variety of reasons, and we appreciate your understanding when changes to the schedule need to be made.

Electronic Medical Record

Our goal as your hospice care team is to ensure that we are providing our clients with quality care. When members of the health care team come for a visit, you may notice them using a laptop computer, iPad, or other electronic devices. At A Peaceful Journey Hospice our health care team charts in an electronic health record which immediately updates the client’s electronic chart. This allows all other health care team members to have up-to-date information at all times. When using the electronic documentation method, members of the health care team must log in to a secure website by entering their user name and password. This ensures that all personal healthcare information is secure. All members of A Peaceful Journey Hospice care team follow the HIPAA regulations. If at any time you have questions or concerns about electronic documentation, please ask and we would be happy to discuss this more.

Length of Hospice Care

Hospice care is provided in benefit periods. Hospice services can be provided for two 90-day periods followed by an unlimited number of 60-day benefit periods. At the start of each benefit period, the medical director or another hospice physician must re-certify that the client is terminally ill and continues to be eligible for hospice services for hospice to continue.

Hours of Operation

Office hours are Monday through Thursday from 10:00 am to 4:00 pm except during company Holidays.

After-hours coverage is provided through 24/7 on-call service, the office phone number forwards to an on-call nurse.
